I think a disease call dropsy looks like that w/the pine coning effect of the scales. Never had one like that and not sure there is a cure. Others will chime in on what you can do for it hopefully.

Dropsy doesn't last years. It looks like the scales are raised. I have female koi that are always fat but if the scales are raised like a pine cone and this is recent, it could be drops.

I agree, I've not heard of dropsy lasting years. The "fatness" appears symmetrical .....could she be egg bound or extremely fat.... ( a bit unlikely ) ? How do your other fish appear?
