Hole in head?

Hi all. I'm attaching a photo of my fish she seems to have gotten this gouge just above her mouth. Any ideas. None of the other fish (koi, sarassas, gold fish) seem to have it. If you need more pics let me know.

I wonder if it is an ulcer from an injury or illness? Or a wound from a heron (that missed!). Just guessing. I hope it gets better!

I had to laugh at your title - we have a fish we call "Old Hole in the Head". Every spring he shows up with a gouge in his head, like he keeps running into something in the pond. Not sure what it is, but it's always in the same place. It heals up pretty quickly, so we just keep making fun of him.
