Looks like a predator either speared it with their beak or clawed it in an attempt to remove it from the pond.
Hospital tank with salt, starting at .3% reduceing daily to .1% for a week. You might also consider antibiotics for the infection.
Make sure you secure the top so it doesn't jump out. I lost a Kohaku on day 6 of this treatment when she jumped out and died.
I was about to release her back to the pond when I discovered her on the floor.

This is a Shiro Utsuri in case you wanted to know.

I just lost one the same way to a raccoon. Mine looked like a twin to your Koi. Before I could treat him, he died.

The treatment I was going to use is:
For abrasion, infection and parasites .6% salt for one week decreasing to .2% followed by Melafix treatments for another 7 days.

For a mature koi, what does ‘a hospital tank’ look like?
I’ve asked the mods to start you a new topic for this question. It’s an excellent question, but this thread is about 9 years old, so many people might miss your specific question. You will get some good answers! We all have or have had a QT (quarantine tank, hospital tanks) at some point.
