Wart like growths on goldfish tail

Hi! I've had my goldfish for about 5 years and a couple months ago she acquired white wart like growths on her tail - I have tried medication that is anti bacterial/fungal/viral/fungal and it hasn't worked - Any idea as to what it might be and how to cure it? : (

Welcome! Is this fish in a tank or a pond? Are there other fish? Any fish added recently?

She is in a 40 gallon Aquarium - she is the only fish (8" goldfish) I have the tank serviced 1x month and use an electric vac. daily - and the nitrates are also high : ( about 60ish
