What is wrong with my Angel?

No-one wants to see a fish lost or for that matter lose one of their own. There are certain items that should be in every Koi owners arsenal. The things I keep are:
1. Salt
2. Scale
3. Tetracycline
4. Parasite meds
5. Dechlor
6. Melafix
7. HT set up and ready
8. 55 gallon drum of treated water
9. Portable microscope
10. Airpumps and airstones
11. Circulation pumps 190GPH
12. Camera, digital
13. TDS meter
14. Test strips
15. Shortcuts and bookmarks to web sites with information

I could go on, DoDad has a very good list as well.
